In order to qualify for ABA services, your child must have a diagnosis of Autism Spectrum Disorder (F84.0). This can be achieved by making an appointment first with your PCM, and then with a pediatric doctor who can evaluate and diagnose Autism.
Make sure you and your child are enrolled in ECHO and the Exceptional Family Member Program. Click the link for more information https://tricare.mil/plans/specialprograms/echo?sc_database=web
If you need assistance, reach out to your PCM, Autism Service Director, or Tricare directly.
Your child will need a referral directly streamlined for ABA to Sprout and Learn for initial assessment. Once we receive the official referral, we can schedule an intake assessment. For assistance, please contact and consult your child's PCM directly.
Once Sprout and Learn has received a referral, it will be time to schedule our initial assessment which is done in person at our clinic in Radcliff.
Complete four (4) assessments required by Tricare East. These assessments will be sent to you via email from Sprout and Learn after completion of the intake assessment. Once the assessments are complete you will be given the treatment plan for ABA to review and sign. Please review the required assessments below
The Parent PDDBI Extended Form (redo every 6 months)
The PSI-4 Short Form OR SIPA (redo every 6 months)
The Vineland-3 (redo every 12 months)
The SRS-2 (redo every 12 months)
Review and sign the treatment plan! It is important to become familiar with measurable treatment goals and to make sure all demographic information is correct! The treatment plan will be updated every 6 months.
Once the treatment plan is reviewed and signed by parent/guardian, the ABA supervisor will submit the authorization for approval to Tricare directly. Once Tricare approves the authorization, we can work out a schedule and begin!
